In order to make it easier to do business with us, we’ve developed one place to request marketing materials needed for quoting and selling UnitedHealthcare products.

Select the type of order you're requesting:

  • Custom Kit Order
    This form allows you to order group/plan specific enrollment kits.
     
  • Bulk Supply Order
    Bulk kits include general enrollment materials (welcome brochure, enrollment form). Bulk kits DO NOT include plan specific information such as Benefit Summaries.

    This bulk order form allows you to order any other loose collateral items in any quantity.
